The standard Medicare Part B premium for 2026 is $202.90 a month ($2,434.80 a year), up from $185.00 in 2025, and the Part B deductible is $283 (2025: $257) (CMS, 2026 Medicare Parts A & B Premiums and Deductibles (14 November 2025)). It is based on income: if the modified adjusted gross income on your 2024 tax return was over $109,000 (single) or $218,000 (joint), you pay $284.10 to $689.90 a month instead, plus a Part D surcharge of $14.50 to $91.00.

Which income sets your 2026 premium
SSA uses the modified adjusted gross income from your tax return for the year two years earlier, so the 2026 premium is set by your 2024 return (20 CFR §418.1135 (the tax year two years earlier)). Modified adjusted gross income here is your adjusted gross income plus tax-exempt interest and a few other excluded items (20 CFR §418.1010 (modified adjusted gross income)).
