CMS has not announced the 2027 Part A and Part B amounts; the cells marked “Trustees estimate” are the 2026 Medicare Trustees Report’s intermediate estimates (Tables V.E1 and V.E2), which the Trustees say these values are estimates, and the actual amounts are likely to be somewhat different as experience emerges. How each deductible works

Part A is charged per benefit period, not per year: a period starts the day you are admitted and ends after 60 days in a row without inpatient or skilled nursing care, so two hospital stays in one year can mean two deductibles. Days 61–90 cost $434 a day, and the 60 lifetime reserve days $868 a day; skilled nursing days 21–100 cost $217 a day (CMS, 2026 Medicare Parts A & B Premiums and Deductibles (14 November 2025)). The coinsurance amounts are fixed fractions of the deductible by law (42 U.S.C. §1395e (Social Security Act §1813)).

Part B is a calendar-year deductible: after the first $283 of Medicare-approved Part B costs you generally pay 20% (Medicare.gov, Medicare costs (what you pay in 2026)). Part D plans may set a lower deductible or none; out-of-pocket drug costs stop at $2,100 in 2026 (CMS, CY 2026 Rate Announcement (7 April 2025), Table V-2 Part D benefit parameters). Medigap high-deductible Plan G is open to people new to Medicare on or after January 1, 2020 (CMS, Medigap high-deductible Plans F, G and J: deductible for 2026 (October 2025)).

Paid tool Medicare costs are set by income you report two years earlier, so the real decision is which years to take income — a Roth conversion, a large IRA withdrawal, a sale — and how far to go before an IRMAA tier makes the next dollar cost more. The Roth Conversion Planner in Personal PRO works that schedule year by year: the conversion in each year, the federal and state tax it costs, and the IRMAA tier that year’s MAGI sets two years later, beside the same years with no conversions. A planning tool, not advice: the numbers move when your assumptions do. Plan my conversions around IRMAA — Personal PRO ($49) → See what is included →
