How IRMAA works in 2026

IRMAA is an income-based surcharge added to your standard Medicare Part B and Part D premiums. It is set by your Modified Adjusted Gross Income (MAGI = adjusted gross income + tax-exempt interest) using a two-year lookback: your 2026 premiums are determined by your 2024 tax return. The brackets are a hard cliff — one dollar over a threshold moves you into the next tier for the entire year, so income events like Roth conversions, capital gains, and required minimum distributions are worth timing carefully.
