Original Medicare (Parts A and B) has no yearly limit on what you pay out of pocket unless you have supplemental coverage such as a Medigap policy, or join a Medicare Advantage plan (Medicare.gov, Medicare costs (what you pay in 2026)). Medicare Advantage, Part D and Medigap premiums are set by each plan. The Part A coinsurance amounts are fixed fractions of the deductible by law (42 U.S.C. §1395e (Social Security Act §1813)), and the IRMAA tiers are set by your 2024 tax return (20 CFR §418.1135 (the tax year two years earlier)).
