You pay more than the standard amount only if the modified adjusted gross income on your tax return from two years earlier crosses the first income-related threshold, $109,000 for a single filer or $218,000 on a joint return. At that first tier Part B becomes $284.10 a month, and at the highest tier $689.90.

Most people pay the standard premium: the surcharge tiers begin well above $109,000 of income. The last column is Part B only — a separate Part D surcharge applies on top at each tier, and on a joint return each spouse enrolled in Medicare pays their own premium and their own surcharge.
Two mechanics decide which row you are on. The income is the one from two years earlier, so a 2026 premium is set by the 2024 tax return, and a one-off spike raises the premium for a single year before falling away. And the tiers are cliffs rather than ramps: clearing a floor by one dollar moves the whole year into the higher row.
