How much do Medicare premiums go up at $200,000 of income in 2026?

QuantCalc · Medicare / IRMAA · 2026 rules · v2026.1

On a joint return, nothing: the first 2026 IRMAA threshold for married filing jointly is $218,000, so $200,000 of modified adjusted gross income still pays standard Medicare premiums. A single filer at $200,000 pays $4,620 more per year — $385 a month across Part B and Part D. The income that counts is the one on the tax return from two years earlier.

Each figure is the whole-year Part B plus Part D surcharge for one enrolled person; the last column doubles it for a couple who are both on Medicare. The surcharge is a cliff — clearing a threshold by a single dollar moves the entire year into the higher tier — and it is re-set every year from the return filed two years earlier, so a one-off income spike raises premiums for one year and then falls away.

Methodology

Every surcharge computed with annualSurcharge() from QuantCalc's parity-tested IRMAA module, reading the 2026 CMS tier table (IRMAA_TIERS). See the 2026 IRMAA bracket dataset, the IRMAA cliff study, and the full 2026 threshold table.

Every figure on this page is computed at build time from QuantCalc's parity-tested calculation modules or a committed dataset — never hand-entered. Reference for research and planning only; not tax, legal, or investment advice.
