Each figure is the whole-year Part B plus Part D surcharge for one enrolled person; the last column doubles it for a couple who are both on Medicare. The surcharge is a cliff — clearing a threshold by a single dollar moves the entire year into the higher tier — and it is re-set every year from the return filed two years earlier, so a one-off income spike raises premiums for one year and then falls away.
