Delaying Medicare is sometimes free and sometimes very expensive, and the difference is a handful of enrollment rules. If employer group coverage from current employment carries you past 65, a special enrollment period lets you enroll later with no penalty at all. Without that protection, Part B adds 10% of the standard premium ($202.90/month in 2026) for every full 12-month period of delay, and Part D adds 1% of the national base beneficiary premium ($38.99 in 2026) for every uncovered month — both for as long as you hold the coverage. This study computes the full grid of 121 delay scenarios through the same yardsticks that power QuantCalc's late-enrollment penalty calculator, then turns each penalty into a lifetime cost at four longevity horizons.
The headline: each full 12-month period of Part B delay buys a $20.29/month surcharge that never goes away, and Part D's meter runs monthly rather than yearly. A 24-month gap on both parts costs $49.98/month ($40.58 Part B + $9.40 Part D) — $599.76 a year, $13,794 by age 90 at 2026 premiums. A five-year dual gap runs $124.85/month and $29,964 by 90; at ten years the Part B penalty reaches 100% — $202.90/month, a doubled premium. Because both CMS yardsticks reset upward most years and the penalty is a percentage of the future yardstick, every lifetime figure here is a floor, not a forecast.
How the two penalties work
The two parts run on different statutes and different clocks, and the difference is worth being precise about:
- Part B (42 U.S.C. §1395r(b), implemented at 42 CFR 408.22): the standard monthly premium is increased by 10% for each full 12-month period you could have been enrolled but were not. Partial periods are discarded — 23 months of delay is one period, 24 is two. Applied to the 2026 standard premium of $202.90, each full period adds $20.29/month, permanently. The penalty is computed on the standard premium — before any income-related surcharge (IRMAA) is stacked on top.
- Part D (42 U.S.C. §1395w-113(b)): 1% of the national base beneficiary premium for every full month without Part D or other creditable drug coverage, rounded to the nearest $0.10 and added to whatever your plan charges. The 2026 base is $38.99, so each month adds about $0.39. The percentage is permanent; the dollar amount re-prices every year as CMS resets the base.
Both figures are read from the same shared, parity-tested constants that drive the live penalty calculator, and this study's arithmetic is pinned to that page's own worked example to the cent (see Methodology). The standard premium figure also appears on our Part B premium answer page and the 2026 IRMAA bracket tables.
The cost of delay, year by year
The centerpiece table: a person eligible at 65 who delays both Part B and creditable drug coverage by whole years, with no special enrollment period covering the gap. Monthly penalties are permanent; the lifetime columns hold 2026 yardsticks constant from enrollment to each horizon age — a floor on the real cost, since both yardsticks reset upward most years.

Why the lifetime curve bends. Two opposing forces are in the lifetime columns. Each extra year of delay raises the monthly penalty (Part B by $20.29, Part D by $4.70 at 2026 rates) — but it also shortens the stretch of life the penalty is paid over, because you were not enrolled during the delay itself. At these horizons the first force wins everywhere in the grid: the lifetime cost rises with every additional year of delay at every horizon. The bend matters at the short end, though — to age 90, the second year of delay adds $6,597 of lifetime cost on top of the first year's $7,197.
The stair and the meter: why 23 months costs less than 24
Part B counts only full 12-month periods — a stair that steps up once a year — while Part D's meter ticks every month. The seam between them decides real cases: a 23-month gap carries a 10% Part B penalty, one more month makes it 20%; meanwhile Part D climbed all 24 of those months, one point at a time.

When a delay is free: creditable coverage and the enrollment windows
Everything above assumes no enrollment protection covered the gap. The protections are specific, and they differ by part:
- The initial enrollment period (42 CFR 407.14) is the 7-month window around the month you first meet eligibility — typically 3 months before to 3 months after the month you turn 65. Enroll inside it and no penalty clock ever starts.
- Working past 65 with employer coverage. If you (or your spouse) stay on an employer group health plan by reason of current employment status (42 CFR 407.20), Part B can wait: once that employment or coverage ends, an 8-month special enrollment period opens, and enrolling inside it carries no Part B penalty. This is the standard path for semi-retirement — the months on the employer plan simply never enter the penalty count. Medicare.gov's sign-up guidance (retrieved 2026-08-12) states the window plainly: it ends “8 months after the group health plan coverage or the employment ends, whichever happens first.”
- COBRA is the trap — on the Part B side only. Continuation coverage is not coverage by reason of current employment (42 CFR 411.104 defines “current employment status” and excludes it), so COBRA does not extend the Part B window; Medicare.gov's guidance is blunt: “COBRA isn't considered group health plan coverage.” The 8-month clock starts when the active employment coverage ends. An 18-month COBRA run therefore lands about 10 months past the deadline — a 10% permanent Part B penalty ($20.29/month, $5,722 to age 90, $6,939 to 95). For Part D the same COBRA months usually count as creditable drug coverage (the test at 42 U.S.C. §1395w-113(b)(4)–(5) is actuarial, not employment-based), so the classic COBRA retirement produces a Part B penalty and no Part D penalty — exactly as the live calculator's guidance walks through. The plan's annual creditable-coverage notice is the document that settles it.
- The Part D window is 63 days, not 8 months: creditable coverage stops the Part D clock while it lasts, but once it ends you have 63 days to enroll before uncovered months start counting.
- Exceptional-conditions relief exists but is narrow: the 2023 SEPs at 42 CFR 407.23 (disaster, employer misinformation, incarceration release, Medicaid termination, other exceptional conditions) carry no penalty when used within their window, per 42 CFR 408.24's exclusion of those months from the count.
In grid terms: months covered by one of these protections never enter the count, so a protected delay sits on the $0 row no matter how long it runs. The grid prices the unprotected months.
The longevity anchor
How long a permanent surcharge is paid matters as much as its size, so the dataset carries four horizons per row. Three are plain set ages — 85, 90, 95 — chosen for legibility, not derived from mortality data. The fourth uses the IRS Single Life Expectancy table (26 CFR 1.401(a)(9)-9(b), as revised by T.D. 9930), which assigns a 65-year-old 22.9 further years — a horizon of age 87.9. That table is the unisex schedule federal regulations use for inherited-account distributions, read here from the same engine table behind our Single Life Expectancy dataset; it is a regulatory life-expectancy schedule, not an SSA actuarial forecast of any person's lifespan, and it is used only as a neutral middle anchor between the set horizons. No mortality weighting is applied — each column is simply the penalty stream from enrollment to that horizon.

Assumptions
- 2026 yardsticks, held constant. The standard Part B premium ($202.90) and the Part D national base beneficiary premium ($38.99) are the 2026 figures published by CMS, held constant in nominal terms for every horizon year. Both are reset annually — and because the penalty percentage applies to each future year's yardstick, the real dollar penalty drifts upward over time. Every lifetime figure here is therefore a floor.
- Eligibility at 65, delay counted from there. The grid takes the count of unprotected months as its input, exactly as the statute does; months covered by employer coverage from current employment, an SEP, or other creditable coverage never enter the count (see the enrollment-windows section above).
- Both parts delayed together in each row. The main grid sets the Part B gap equal to the Part D gap. Because the two penalties are computed and shown separately, a one-part gap (the COBRA shape: Part B late, Part D covered) reads directly from the individual columns.
- Lifetime cost = penalty stream only. Each lifetime column is the monthly penalty times the months between enrollment (65 plus the delay) and the horizon age. The premiums themselves, IRMAA surcharges, plan premiums, and any medical costs paid out of pocket while uninsured during the gap are all deliberately excluded — the study prices the penalty, not the whole decision. The out-of-pocket exposure during an uninsured gap can dwarf the penalty and argues in the same direction.
- No discounting, no inflation adjustment. Figures are nominal sums at 2026 rates. Discounting future penalty payments would shrink the totals; annual yardstick growth would raise them.
- Part D's 1% is the operative minimum. The statute sets the penalty at the greater of an actuarially determined amount or 1% per uncovered month; CMS practice has used 1%, which is what this grid (and every published example) applies.
- IRMAA is out of scope here — the penalty applies to the standard premium and income-related surcharges stack separately on top (above $109,000 MAGI single, they begin). The 2026 IRMAA brackets page carries every tier.
Methodology
Yardsticks: the two dollar inputs — the 2026 standard Part B premium and the 2026 Part D national base beneficiary premium — are read from frontend/js/calc/irmaa.js (BASE_PART_B_2026, PART_D_BASE_2026), the same shared constants behind the live penalty calculator; the longevity anchor divisor comes from constants-2026.js's SINGLE_LIFE_TABLE, the transcription of T.D. 9930 behind the Single Life Expectancy dataset. No premium figure is typed into this generator.
Penalty arithmetic: Part B adds 10% of the standard premium per full 12-month period (partial periods discarded); Part D adds 1% of the national base beneficiary premium per uncovered month, rounded to the nearest $0.10. This reproduces the live calculator's own function, and the generator refuses to write anything unless its output matches that page's published worked example (24+24 months → $40.58 + $9.40 = $49.98/month, $599.76/year, $14,994 over 25 years) to the cent.
Sanity gates: seven built-in checks run before anything is written: (1) zero delay produces exactly $0 everywhere; (2) a longer delay never lowers a penalty, and Part D rises every single month; (3) a longer horizon never lowers a lifetime cost, and strictly raises it while a penalty is being paid; (4) Part B is an exact step function of full 12-month periods; (5) the live-page worked example reproduces to the cent; (6) every lifetime figure is non-negative and equals monthly × remaining months; (7) Part D's rounded figure stays within $0.05 of the exact 1%-per-month amount. Three deliberate mutation runs (continuous Part B accrual; unrounded Part D; a flipped remaining-months sign) are part of the release process and must each trip the gates.
Cross-checks: 14 assertions rebuild, from the engine constants and this generator's own arithmetic, figures that already appear on /medicare-late-enrollment-penalty/, /irmaa-brackets-2026/, /answers/medicare-part-b-premium/ and /data/single-life-expectancy-table/, and require each live page to still print exactly those values before this study can be generated — including two verbatim locks on the calculator page's COBRA statements, so this study cannot drift from the enrollment-rule copy already reviewed there.

Not advice: this is educational research on the statutory penalty schedule, not a recommendation about anyone's enrollment. Individual timing turns on facts outside this grid — confirm your own windows with SSA/Medicare or a qualified professional.

The penalty applies to the standard premium — but what you actually pay two years from now also depends on your MAGI, because IRMAA surcharges stack on top and a single large Roth conversion can cross several tiers at once.
